Scan your documents and e-mail both the signed license release form and proof of business ownership by replying to your e-mail order confirmation that you received when you placed your first order.
Please fill out the form completely with your order number and submit the correct documentation as that it will be considered void if it is incomplete.

PROCEDURE: Prior to purchasing  an X-Mix product you must return this signed form to Rotations with either ONE of the following:

• A copy of your business card that represents your entertainment business or
• A page that has your published company advertisement or
• A copy of your entertainment business contract or
• A copy of your business registration license or
• A business tax ID form or
• A copy of the venue's liquor license.

I wish to become a purchaser of X-Mix and compilation discs. I understand that I may use these discs only in connection with my professional entertainment business activities, or at my venue. I understand that X-Mix Music CD compilations are licensed only for promotional use by professional entertainment businesses, clubs and public performance venues and that any other use, including unauthorized resale, distribution, public display, duplication, violation of applicable laws, including federal copyright laws, or any other use is strictly prohibited. Any person or business who purchase X-Mix discs that violate these conditions will be subject to any legal actions resulting from such misuse, including but not limited to, criminal prosecution. All respective copyrights of the sound recordings and musical works embodied on X-Mix CD’s are the property of participating artists, composers, authors, publishers and record labels as applicable. I hereby certify that I own an entertainment business, club or public performance venue and that all X-Mix CD’s will be used only in that capacity. I further acknowledge and agree that all music programs are acquired on the basis that they may not be resold, exchanged or traded in any way and that any and all applicable ASCAP, BMI, SESAC or other related licenses are not the responsibility of X-Mix.  Rotations and X-Mix reserves the right to limit or refuse the sale of any X-Mix product. Verification of your eligibility may be verified in conjunction with the sales audits submitted by Rotations, X-Mix , or a licensing agency.

I find these terms and conditions satisfactory by my signature and I understand and agree to all of them.
