The completion of this form is required of all current or new clients who wish to qualify to purchase the RockAmerica DVD Video series from Rotations. To ensure a prompt order processing, please read the directions and information on License Release Information Page.  Please submit this form when you submit your order on line for for the RockAmerica DVD Video series. Details on mailing, faxing or e-mailing a scan copy of the completed form are detailed below. Please submit this form only within one business day of your first order with Rotations. We always suggest an e-mail follow up to ensure that we received your DJ Only Release form and order.  Thanks!

Please fill out the form completely with the correct documentation as that it will be considered void if it is incomplete.  A RockAmerica DVD order will not ship until this license release form and business credential documentation have been submitted to Rotations.
Copyright/Proprietary Protection:

These products are owned by RockAmerica or its suppliers, and is protected by the United States and International copyright laws, and International treaties. You must treat these products like any other copyright material. Your right to use these products terminates if you violate any part of this license. If terminated, return all products to RockAmerica.

License Grant:

  • You may use only the original material on one video system. If a system is used at an address other than stated on this license, a multiple venue fee must be arranged.
  • The licensee may use these videos for non-theatrical, non-broadcast, public display as many times as they want during the license period. They may charge any fee to enter, or use, or sell products in their venue under license, provided that no fee is charged directly for the viewing of any license material.
  • The licensee may publicly promote information concerning RockAmerica videos in all mediums provided the license material is not directly used, except with written permission from RockAmerica.
  • By accepting any of the license material from any carrier, mail, messenger service, personal delivery of any sort, or by any other means obliges the licensee to this subscription, and license agreement.

License Restrictions: Licensee may not:

  • Rent, lease, loan, or sell any license material.
  • Use tapes unless they hold all required public performance music licenses
  • Duplicate either the audio, video, or both portions of the license material.
  • Use any segment of any program if requested by either RockAmerica, or its suppliers.
  • Retain the tapes past the end of the contract period without renewing this agreement.

You need to choose only one of the following written criteria that is an acceptable verification of your entertainment business or venue ownership. You may send via scan & e-mail, fax or mail your choice of one listed documentation with your RockAmerica License Release Form and order form. Please send only one of the four forms of proof of business:

  • A copy of your business card or

  • A page that has your published company advertisement or

  • A copy of your business registration license or

  • A tax ID form
