SCOPE: Every professional Entertainment Business owner, club or other public access venue who intends to purchase promotional only product for their entertainment business from Rotations must, and without exception sign and submit a release form to Rotations.

PURPOSE: It is the intention of the entertainment promotional CD/DVD compilation vendors to prevent unauthorized, non professional, casual use, exchange and the resale of their product to other individuals or to a retailer. This is also to prevent infringement of the owner's copyright as protected by the copyright laws of the United States.

PROCEDURE: Prior to purchasing any entertainment promotional CD/DVD compilation  for your business, you must return this signed form to Rotations with either any one of the following standard business credentials:

Rotations and or our entertainment promotional product  product vendors reserves the right to limit or refuse the sale of any of their CD/DVD product. Verification of your eligibility may be verified in conjunction with the sales audits submitted by Rotations to any requesting vendor, licensing or government agency.

TO WHOM IT MAY CONCERN: I wish to become a purchaser of entertainment only promotional CD/DVD compilation discs for my professional entertainment business, club or public access venue. I understand that I may use these discs only in connection with my professional entertainment activities, or at my club or public access venue.  I agree by my signature that these discs may not be resold or exchanged for any purpose whatsoever. I acknowledge that selling or trading the entertainment promotional CD/DVD compilation discs to retailers or any other individual may constitute a violation of federal and state law, and infringement of the record master owner's copyright and may subject me to civil and criminal penalties. I find these terms and conditions satisfactory by my signature and I understand and agree to all of them.
