Rotations 3 Step Process For New Clients:

1. Place your order on line.  You will need your order information for the License Release Form. Note in the Special Instructions section of of the order form when you are sending in your License Release Form.  Ideally, we would like to receive your License Release Form within one business day of submitting your order.
2. Choose the form you need below according to the vendor you purchased.  Multi vendor orders need only one form.
3. Fax, Scan Or E-Mail the completed License Release Form    to us within one business day of sending us your order. Our mailing address is at the bottom of our web pages and forms. • Your entertainment business or venue is listed in your local yellow pages and that the company and address matches the billing/shipping information on your order form.

General Information About The License Release Forms
This is general background information for Rotations' License Release Form to purchase restricted specialty product designed for promotional use only by venues or a professional entertainers in their business activities. The entertainment only audio product are:

Rotations is required to verify that we are selling the above listed product to qualified professional entertainers, business owners of venues who reside in the United States. Rotations, as a reseller, is under contractual agreement by the manufactories of the entertainment only product to confirm the eligibility of each new buyer of their product and will use the product for professional entertainment business activities. This completed License Release Form and a proof of business document (usually your entertainment business card)  is required first if you are purchasing from Rotations Monthly CDs, Remix CD's  or DVD Video product that is licensed for professional entertainment use only. 

This process of submitting an easy License Release Form as well as your verification of business ownership is standard business procedure in the promotional industry.  It is required in the legitimate marketplace when purchasing licensed, promotional compilations from a reputable vendor that adheres to their licensing agreements. You can rest assured that you are purchasing licensed, promotional product intended for your professional business activities at your entertainment business or venue.

The DVD Series by RockAmerica has a tighter restriction by the manufactures and record labels.  A written RockAmerica Video License Release Form must be submitted before we can ship any DVD Video Series.  That form is submitted and your business documentation is submitted for you by Rotations to respective vendor  That information is noted below DVD Videos from ERG.

We have stream lined this process so you can easily send the information via e-mail for the music product once you place your order on line for the music product.  

Save your valuable time without sending in any written documentation by submitting your proof of entertainment business in one quick step on the order form process. If you can fulfill one of the three follow criteria, you will not have to send in any of the written documentation in including the License Release Form. This method is the easiest way to qualify your business to purchase CD product that is licensed for professional entertainment use only. Sending Your License Release Form Information Via Fax, Scan Or Mail (Monthly And Remix CDs)
You need to choose only one of the following written criteria that is an acceptable verification of your entertainment business or venue ownership. You may send your choice of one listed documentation with your License Release Form and order form. Please send only one of the four forms of proof of business:

• A copy of your business card or
• A page that has your published company advertisement or
• A copy of your business registration license or
• A tax ID form

The fax method is the easiest and quickest way to submit your License Release Form and probably your business card of your entertainment business or venue. Please fax us your License Release Form once you place your on line order.  In most cases, your order can ship within one business day (if not the same day before 1 PM Eastern Time) after submitting your License Release Form.  To ensure that we receive your fax, we suggest you also submit your order form via our secure on line order system. Please note in the Special Instructions section at the bottom of the order form the date and time you faxed us the information. Doing so will ensure that we can find your License Release Form and begin to process your order. Please monitor your e-mail. We will confirm your order, update you on stock status and give you a tentative date of shipment within 24 hours of the next business weekday.

All purchasers of the DVD Video series by RockAmerica and ScreenPlay are required to submit a written RockAmerica Video License Release Form as required by the recording industry. There is currently no permissible method of submitting your RockAmerica Video License Release Form or a via an e-mail procedure. Your RockAmerica Video License Release Form form will forwarded on your behalf to RockAmerica once we ship your DVD Video order.
