The e-mail license release form is designed to submit on line your entertainment proof of business ownership to purchase Monthly CD Compilations and or CD Remixes.  Please submit your first order on line prior to completing this form.  Our DVD Video series require a written DVD License form.

Definition Of An Entertainment Business:  Rotations defines the term "Entertainment Business" as the following establishments: Venues, Bars, Clubs  as well as Karaoke or Mobile Entertainment Businesses who perform in these facilities.  Only a professional entertainment business is eligible to submit this form for the purchase of Monthly Compilations, DVD Videos and Remix Compilations.

AUDIO LICENSE RELEASE FORM

SCOPE: Every professional entertainer, entertainment business or venue who intends to purchase licensed promotional product intended for business use only from Rotations must, and without exception sign and submit a release form to Rotations.

PURPOSE: It is the intention of those vendors who manufacture this product to prevent unauthorized, non professional, casual use, exchange and the resale of their product to other individuals or to a retailer. This is also to prevent infringement of the owner's copyright as protected by the copyright laws of the United States.

PROCEDURE: Prior to purchasing any DJ only product CD, you must return this signed form to Rotations with either:

• A copy of your business card (preferred) or:
• A page that has your published company advertisement or:
• A copy of your business registration license or:
• A tax ID form

Rotations and or our product vendors reserves the right to limit or refuse the sale of any product. Verification of your eligibility may be verified in conjunction with the sales audits submitted by Rotations to any requesting vendor, licensing or government agency.

TO WHOM IT MAY CONCERN: I wish to become a purchaser of compilation discs that is licensed promotional product intended for business use only. I understand that I may use these discs only in connection with my professional business activities, and that these discs may not be resold or exchanged for any purpose whatsoever. I acknowledge that selling or trading promotional product discs to retailers or any other individual may constitute a violation of federal and state law, and infringement of the record master owner's copyright and may subject me to civil and criminal penalties. I find these terms and conditions satisfactory by my signature and I understand and agree to all of them.
